About the Role
The Child and Adolescent Community Health Service is seeking a suitably qualified and experienced Community Health Nurse to join our multidisciplinary Refugee Health Team (RHT) based in Midland. You will join a supportive, multidisciplinary team committed to culturally safe, equitable and high-quality community care.
This role focuses on improving the health and wellbeing of children, young people and their care providers, through evidence‑based health promotion, education, and early intervention strategies that address the social determinants of health.
You will provide high‑quality preventative and early intervention programs to individuals and groups, with a strong emphasis on supporting refugee young people and their caregivers during their early settlement period as they establish their lives in Western Australia.
Working collaboratively as part of a multidisciplinary team, you will support clients to navigate the health system and strengthen their understanding of health rights, including use of interpreters to support health literacy, and improve access to appropriate health services.
The role requires the ability to work both autonomously and collaboratively within the multidisciplinary RHT and broader Community Health Services.

About Child & Adolescent Community Health (CACH): Our teams provide health and developmental assessments, early intervention and health promotion services for children and young people across the Perth metropolitan area.
These services delivered at community health centres, schools, homes and through telehealth appointments with a focus on child health, growth and development, behaviour, immunisation and family support.
