The Transplant Australia Social Worker (WA Families) role is a funded role to support the delivery of a pilot program supported by Telethon that will provide holistic, person-centred support to children and families in Western Australia navigating the transplant journey.

This role focuses on bridging the gap between hospital-based care and community based support, ensuring families feel connected, informed, and supported at every stage. This role will also contribute to the development of Transplant Australia family recourses for ongoing support. This is a pilot position, currently funded until December 2026, designed to test and evaluate a community-based support model for transplant families in WA. Insights and outcomes from this role will inform future program development and potential expansion.

Key Responsibilities

1. Individual & Family Support

  • Provide one-on-one, strengths-based support to children and families throughout the transplant journey

  • Assist families to navigate non-clinical support services including community, education, financial and wellbeing supports

  • Act as a trusted point of contact for WA families, offering guidance beyond the hospital system

  • Identify emerging needs and provide early intervention support where appropriate

2. Care Pack Program Delivery

  • Coordinate and deliver care packs to applicable WA child entering the transplant journey

  • Support logistics, inventory, and continuous improvement of pack contents

3. Peer Mentor Support

  • Facilitate connections between children and young people through supporting organised Transplant Australia activities

  • Support planning and delivery of local family camps

  • Monitor and guide mentor relationships to ensure safe, positive engagement

4. Resource Development (Digital & Family-Focused)

  • Contribute to the development of accessible, practical online resources for families

  • Identify gaps in available information and co-design content with lived experience input

  • Support the delivery of webinars, guides, and digital tools tailored to transplant families

5. Stakeholder Engagement

  • Build and maintain relationships with hospitals, clinicians, community organisations, and support services across WA

  • Advocate for the needs of transplant families within community and support networks

  • Collaborate with internal teams to align national programs to local WA needs

6. Program Support, Evaluation & Reporting

  • Maintain accurate and confidential case notes and records

  • Track engagement, outcomes, and program impact

  • Contribute to reporting requirements associated with grant funding

  • Support evaluation activities to assess the effectiveness of the pilot program and inform future funding opportunities

Key Selection Criteria

Essential:

  • Qualification in Social Work, Counselling, Youth Work, or related field

  • Strong digital literacy and competency in use of MS Suite • Demonstrated experience working with children, young people, and families

  • Strong understanding of holistic and trauma-informed care approaches

  • Experience navigating community and support service systems • Excellent interpersonal and communication skills

  • Ability to work independently in a part-time capacity while staying connected to a national team

Desirable:

  • Experience working with chronic illness, hospital systems, or transplant related care

  • Experience coordinating and delivering events and/or community activities

  • Experience working with volunteers and community groups

  • Understanding of peer support or mentoring models

  • Experience in resource or program development (particularly digital content)

  • Lived experience of transplantation (personal or family)
