About the Role
Plan and Grow is seeking experienced, passionate, caring Support Workers with English proficiency and full availability to assist in supporting participants with disabilities.
We are looking for candidates who possess strong boundaries, mentorship skills, and experience in this field, especially with behaviours of concern and de-escalation strategies.
This role will be working across both the In-Home and Social division and our 24/7 SIL houses. Locations North and South of the river.
Key Responsibilities:
Support participants in reaching their full potential through both practical and emotional support, assisting them to actively work towards achieving the goals outlined in their NDIS plans.
Carry out domestic duties where required to ensure the home is kept clean and tidy, including but not limited to household cleaning, laundry, tidying, and gardening.
Accompany participants on outings, including appointments, shopping, social, community, and leisure activities as per scheduled plans.
Support participants with medical and therapy appointments, ensuring all relevant documentation is provided to the direct line supervisor.
Ensure food and groceries are adequately maintained, shop with participants, and prepare meals as required.
Complete and submit all required documentation within designated timeframes, including Maintenance Requests, Progress Notes, and Incident Reports.
Ideal Candidate:
Proven experience working with disability.
Strong communication skills with the ability to engage clients with empathy, respect, and professionalism.
Ability to work effectively with young people, demonstrating patience, resilience, and understanding.
Knowledge of de-escalation techniques and managing behaviours of concern.
Flexible, with the ability to adapt to varying work hours and responsibilities.
Interest in gaming or familiarity with gaming culture is highly regarded for this specific participant.
Essential Requirements:
Proficient in both written and spoken English.
NDIS Worker Screening Check
Working with Children Check
2–3 years’ experience or relevant qualifications in the disability sector
NDIS Induction and Orientation Modules (including Supporting Effective Communication, Incident Reporting, Infection Control, etc.)
Valid Driver’s Licence and reliable vehicle
Additional training may be required for successful candidates prior to commencement.
